Definition:

When everything that could go wrong goes wrong indeed. (See also Tedious Chaos). It's a disaster situation for work or dating.

Exemplification of usage:

Them: How’s work going?

Me: The right hand doesn’t know what the left hand is doing. It’s a dumpster fire of monumental proportions.
